Referral

Who is Eligible?

We accept guests who meet the following criteria:

• Adults experiencing homelessness or housing instability
• Recently discharged from a hospital or medical facility
• Medically stable yet unable to recover in a shelter or on the street
• Referred by a licensed medical doctor, hospital or clinic

Please ensure potential guests can perform basic activities of daily living and do not require inpatient care. For compliance with state guidelines, referrals must be made by a physician or through a discharging hospital or clinic.

How to Refer

Referrals must originate from a physician or from a hospital or clinic discharge planner. To initiate a referral:

1. Contact us at (612) 450‑0358 or email serenityrecuperative@gmail.com to confirm bed availability.
2. Provide patient demographics, discharge summary and the reason for referral.
3. Coordinate transportation to our facility once acceptance is confirmed.
